Pain and uterine bleeding is the most side effect that make IUD acceptors to stop . No hand and withdrawal technics can’t be done because the size of IUD ( 20 cm long in size of CuT 380A with conventional inserter). The new CuT 380A R-inserter has longer inserter (28 cm in long) and bigger diameter (1mm) that expexted to inserted in right potitions that can reduce the side effect, both of pain and also uterine bleeding. Methods: This study performed in 100 IUD acceptors that divided to two groups. The control group that consist of 50 Conventional CuT 380A IUD and consist of 50 R-inserter CuT 380A IUD acceptors. We analysed the side effect after 1 month IUD insertion. The data analysed in statistic test performed by SPSS (Software Package for Social Science) 21. Results: The pain evidence in conventional IUD acceptors groups are 34 acceptors has no pain, 14 acceptors has mild pain, 1 acceptor has moderate pain, and 1 acceptors has severe pain. For the R-Inserter, they are 41 acceptors has no pain, 9 acceptors has mild pain. We found statistically no significant difference in conventional groups compare to R-Inserter (p=0.70). The uterine bleeding evidence in conventional IUD acceptors are 8 acceptors, and for the R-Inserter, they are only 2 acceptors has uterine bleeding. We found statistically significant difference in conventional compare to R-Inserter (p=0.046*). Conclusion: There are no significant differences of pain in R-Inserter compare to conventional group (p>0.05). There are also significant differences of uterine bleeding evidence in R-inserter group compare to conventional group (p<0.05). The R-inserter group has less uterine bleeding evidence compare to conventional groups. Keywords: IUD, Uterine Bleeding, Pain
