    Abstract

    ACCIDENT INVESTIGATION WITH SYSTEMATIC CAUSE ANALYSIS TECHNIQUE (SCAT) METHOD IN PT. DIC ASTRA CHEMICALS, EAST JAKARTA Ervinna Tiara NP1, Seviana Rinawati2 Background: PT. DIC Astra Chemicals is a manufacturing company engaged in the field of chemical industry and a manufacturer of industrial dyes for plastics, textiles, paint and wood. In the production activities, there are potential hazards that can cause workplace accidents and occupational diseases. In an effort to prevent and control work accidents, company applies an accident investigation process, so the same incident does not reoccur. The purpose of the research is to know the implementation of accident investigation, reporting and analysis of accidents that occurred in the area of mixer production 2 using SCAT method. Methods: This research uses descriptive method to describe the implementation of investigation and reporting of accidents at PT. DIC Astra Chemicals and analyze the accidents that occurred in the area of mixer production 2 using SCAT method. Result: PT. DIC Astra Chemicals has implemented accident investigation and reporting by the early stages of planning, organizing, conducting investigations, and controlling. The results of accident investigation analysis in the area of mixer production 2 using SCAT method are the causes of accident that consist of direct causes include unsafe acts and conditions, and basic causes that consist of human factor and work factor. Conclusion: The implementation of accident investigation and reporting program has been running well and in accordance with the Regulation of the Minister of Manpower of the Republic of Indonesia Number Per. 03/MEN/1998 on the Procedure of Reporting and Inspection of Accidents. Based on the results of the analysis by SCAT method, the causes of accidents in the mixer area consist of the underlying basic cause and the direct cause, which is the company will take control measures that is administrative control by making the procedure about the using of the tank on the area of the mixer.
