Biotech.St, Ph.D, Program Studi Ilmu Gizi, Pascasarjana Universitas Sebelas Maret Surakarta ABSTRAK Latar Belakang: Kanker payudara merupakan salah satu kanker yang banyak diderita wanita dengan tingkat morbiditas dan mortalitas yang tinggi. Serat berperan dalam pencegahan kanker payudara melalui regulasi hormon steroid dalam sirkulasi darah Pasien kanker mengalami hipermetabolisme yang ditandai peningkatan metabolisme energi dari lemak dan berpengaruh terhadap penurunan berat badan pasien. Angka mortalitas dan morbiditas kanker payudara menurun setelah pasien melakukan aktivitas fisik dengan intensitas sedang. Penelitian ini bertujuan untuk menganalisis hubungan antara asupan serat, lemak, dan aktivitas fisik waktu luang dengan profil lipid pada pasien kanker payudara. Metode: Jenis penelitian ini adalah observasional analitik dengan rancangan cross sectional. Subjek penelitian ditentukan menggunakan teknik consecutive sampling. Sebanyak 41 pasien kanker payudara rawat jalan di Rumah Sakit Dr. Moewardi Surakarta bersedia ikut penelitian. Data asupan serat dan lemak diperoleh dengan metode recall 2x24 jam dan FFQ semi kuantitatif. Data aktivitas fisik waktu luang diukur dengan kuesioner Godin Shepard Leisure Time Physical Activity. Pemeriksaan profil lipid dengan metode kolorimetrik. Uji statistik Pearson Product Moment, Rank Spearman dan regresi linier berganda digunakan untuk menganalisis data dengan nilai signifikansi p< 0,05. Hasil: Subjek penelitian memiliki rerata asupan serat (12,86+6,18 g), asupan lemak (56,80+26,41g), kadar kolesterol (206,80+57,54 mg/dL), kadar HDL (49,95+10,60 mg/dL), kadar LDL (132,46+37,97 mg/dL), dan kadar trigliserida (192,63+ 137,77 mg/dL). Hasil uji multivariat menunjukkan pengaruh usia (B=0,590 p=0,735), IMT (B=3,935 p=0,065), stadium (B=10,88 p=0,604), siklus kemoterapi (B=0,610 p=0,830), status menopouse (B=30,910 p=0,271), satatus paritas (B=20,371 p=0,724), jenis KB (B=-10,952 p= 0,569), status menyusui (B=-31,931 p=0,256), asupan karbohidrat (B=0,264 p=0,156), asupan protein (B=-0,33 p=0,950), asupan serat (B=-3,767 p=0,029), asupan lemak (B=0,451 p=0,323), dan aktivititas fisik waktu luang (B=-0,994 p=0,450) dengan kadar kolesterol. Kesimpulan: Asupan serat menurunkan kadar kolesterol pada pasien kanker payudara. Setiap peningkatan asupan serat 1 gram, maka akan menurunkan kadar kolesterol sebesar 3,767 mg/dL. Fibers contribute in breast cancer prevention through regulation of steroid hormones in blood circulation. Hipermetabolism occurs in cancer patients which are characterized by the increase of lipid energy metabolism and result in weight loss. Morbidity and mortality rate of breast cancer are reduced among women who perform moderate physical activity. This study aimed to analyze the relationship between intake of fiber, fat, and physical activity on the lipid profile in breast cancer patients. Method:The study was an observational analytic with cross sectional design. Research subject were determined using consecutive sampling. Forty one outward patients of breast cancer in Dr. Moewardi Hospital Surakarta followed this study. Data of fiber and fat intake were collected used 2x24 hour recall and semi-quantitative FFQ method. Leisure time physical activity was measured by using Shepard Godin Leisure Time Physical Activity questioner. Lipid profile was determined using colorimetric assay. Pearson Product Moment, Rank Spearman, and multiple linier regression were used to analyze data and the signifant value was p< 0.05. Result:The research subject had a mean intake of fiber (12,86+6,18 g), fat intake (56,80+26,41 g), cholesterol level (206,80+57,54 mg/dL), HDL level (49,95+10,60 mg/dL), LDL level (132,46+37,97 mg/dL), and triglyceride level (192,63+137,77 mg/dL). Results of multivariate analysis showed the effect of age (B = 0,590 p = 0,735), BMI (B = 3,935 p = 0,065), stage (B = 10,88 p = 0,604), cycles of chemotherapy (B = 0,610 p = 0,830), menopausal status (B = 30,910 p = 0,271), parity status (B = 20,371 p = 0,724), type of birth (B = -10,952 p = 0,569), breastfeeding status (B = -31,931 p = 0,256), carbohydrate intake (B = 0,264 p = 0,156), protein intake (B = -0,33 p = 0,950), fiber intake (B = -3,767 p = 0,029), fat intake (B = 0,451 p = 0,323), and leisure time physical activity (B = -0,994 p = 0,450) with cholesterol level. . Conclusion: Fiber intake decrease cholesterol level in breast cancer patients. Every 1 gram increase fiber intake, will decrease cholesterol by 3,767 mg/dL.
