    EFFECT OF TEOPHYLLINE ON INTERLEUKIN-8 LEVEL, NETROPHIL SPUTUM, AND COPD ASSESSMENT TEST SCORES IN STABLE COPD PATIENTS. Background: Particles and harmful gases especially from cigarette smoke can induce an inflammatory immune response in the lungs and decrease the function of histone deacetylase 2 (HDAC2) in the nucleus cell, and causing glucocorticoid resistance in COPD. It is associated with the increasement of pro-inflammatory cytokines release including TNF-α and IL-8 which occurs continuously. Theophylline with low plasma concentrations that less than 10 mg / L have anti-inflammatory effects by activating the HDAC. The aim of the study is to analysed effect of theophylline in inflammation response and clinically improvement for stable COPD patient. Method: This study was clinical quasy experimental and performed by using pre test and post test design. The subjects were stable COPD patient who control in outpatient pulmonary clinic Dr Moewardi hospital in July to September 2016. Thirty subjects were taken by consecutive sampling and divided into two groups, treatment groups (n = 15) received additional treatment with theophylline 70 mg per 8 hours and the control group (n = 15) with just standard therapy. The IL-8 levels, neutrophil sputum and CAT scores were measured at before and after 4 weeks of treatment. Result: The IL-8 levels of treatment group decreased 4,31±13,06 pg / ml while the control group increase 0,88±13,89 pg / ml, but it was not statistically significant (p = 0.116). There were a decrease of sputum neutrophil percentage in the both of groups. The treatment group was 4,00±40,33 pg / ml and the control was 2,87±39,91 pg / ml, but it was not statistically significant (p = 0.939). CAT scores in the treatment group decreased 3,67±3,58 pg / ml but the control group increased to 2,40±3,38 pg / ml and statistically significant (p = 0.003). Conclusion: Theophylline can decrease IL-8 levels and neutrophil sputum not significantly, but it can decrease CAT scores significantly.
