    Abstract

The different Effect between paracetamol and ketorolac to trombocyte agregation activity on SIRS or Septic Patients. 1st Advisor: Dr. Untung Alifianto dr, Sp.BS. 2nd Advisor: Eko Setijanto, dr, Msi, Med, SpAn. KIC. institution Program: Family Medicine Magister Program of Sebelas Maret University, Surakarta 2016 Background: Septic is an inflamatory respond to infection and its a common and serious problem on patients in the ICU. The septic mortality presentage is between 25-80% in the world. Paracetamol blocks the sintetic of prostaglandin in cells, blocks cyclooxigenase in central, and works in the periferal pain chemoreseptor. Ketorolac blocks prostaglandin synthesis at peripheral pain receptors by inhibits the formation of tromboxane. Both interaction can effect the trombocyte agregation that can be checked with the trombocyte agregation test. Objective: To understand the differance between between paracetamol and ketorolac to trombocyte agregation activity on SIRS or Septic Patients. Methode: This research was an experimental experiment with clinical approach before and after the experiment. there were 30 SIRS or septic patients in the ICU between the age of 18 – 65 years old as the experiment subjects. Sampling distribution was devided to 15 subjects that were given paracetamol and 15 subjects that were given ketorolac. After we rendomised it, we checked the trombocyte agregation before and 60 minutes after the treatment with an 10 µM ADP inductor. This reaserch was using catagorical data with ordinale scale so the non parametric statistic test was used in the differentiation test. The unpaired differentiation test was using the mann whitney test. Meanwhile the paired differentiation test was using the Wilcoxon test. Result: The result of the differentiation test to the caracteristic of the research subjects we know p > 0,05, this explains that there wasnt any significant difference between the subjects. Based on the Mann Whitney test on the unpaired group, the result was p=0,355, which means there wasnt any significant difference between paracetamol and ketorolac group before the treatment. And the result after the treatment was p=0,098, which means there also wasnt any significant difference. And we did the Wilcoxon test on the paired group, and the result was p=0.705 (p>0,05) which means there wasnt any significant difference of trombocyte agregation between before and after the giving of ketorolac. and p=0.317 (p>0,05) which also means there wasnt any significant difference of trombocyte agregation between before and after the giving of paracetamol. Conclusion: The use of ketorolac more potent inhibition of platelets aggregation in SIRS or septic patientscompared with paracetamol, but satistically unsignificant(p=0.098) Keywords: Trombocyte agregation activity, ketorolac, paracetamol, SIRS, septic
