The data from World Health Organization indicate that there are 140 million teenage females who got married before the age of 18, and 50 million below the age of 15. The reproductive health at marital period includes prenatal care, delivery assistance, and postnatal care. Early marriage has come to the attention of policy makers and program planners, for the high risk of failure in marriages, early pregnancy leading to maternal death, and risks of mental unpreparedness to raise a familyand be responsible parents whichresult in the poorfamily quality. This study was aimed to formulate a model of health promotion in improving reproductive health in married teenagers in Sleman Regencyusing determinant hypotheses: knowledge level of reproductive health, the democratization level of parenting, PIKR counseling, and peer support simultaneously have indirect effects on the reproductive health of married teenage females, mediated with their maturity level. This research employed a survey with cross sectional design. The research samples were 208 married teenage females, using questionnaires as data collection tool. The sampling used multistage proportional random sampling with Structural Equation Model (SEM) analysis with program Amos 20. Goodness of fit model result indicates that chi square is 277.650; probability of significance = 0.063; relative chi-square = 1,143; RMSEA = 0.026, GFI = 0.905; = GFI = 0.879; NFI = 0.879; CFI 0.983. The knowledge level of reproductive health, democratic parenting, PIKR counseling, and peer support significantly affect the level of reproductive health in married teenagers, mediated by the maturity level of marriage. Conclusion: The model of health promotion to improve reproductive health in married teenagers through marital maturity is done by PIKR counseling, reproductive health knowledge, optimizing democratic parenting and peer support.
