This study aims at obtaining: (1) spatial map of water quality base on pollution sources derived from human activities in upper course flowing to Gajah Mungkur Reservoir (GMR) through 7 sub-river basins, which cause decreased water quality in GMR, on the basis of pH, TSS, DO, BOD, COD, N-NO2, N-NO3, N-NH3, P-PO4, Mn, Cu, Fe, Cr, Zn, E-coli and Total Coliform parameters; (2) Determining toxicity levels and dominant chemical compounds and elements in contaminated water quality zones in GMR; (3) Developing water pollution control model for Rainwater Catchment Area of GMR through system dynamics to design recommendation for GMR water source pollution control policy. This research applied descriptive-ex ante analytical approach to analyze policy for a certain phenomenon which had not happened. The research subject was the phenomenon of decreased water quality in GMR as a result of waste load flow of human activities in the Rainwater Catchment Area of GMR and in the reservoir. This study was also a descriptive spatial-laboratorial research which was done to describe existing waste source flow based on human activities in the Rainwater Catchment Area of GMR, through spatial map on the basis of laboratory data analyses. Meanwhile, GMR pollution control system modeling employed Dynamics System methodology with Powersim Studio 10 software. The results indicate that dominant pollution sources contributing to decreased water quality in GMR are farming, domestic activities, tofu industry and floating fish cage. Farming activities are the biggest pollutant contributors with 81.76% of TSS, 11.58% COD, and 90.96% of P-PO4. Organic pollutants are mostly emitted from tofu industrial waste (47.65%) and domestic waste (32.72%). GMR water zone is considered to have moderate to serious contamination quality status, with dominant pollution sources of TSS, BOD, COD, P-PO4, E-coli and Total Coliform. Sediments of contaminated zones contain heavy metals of Cd, Cu, Zn and Pb with SQG-Q values ranging from 0.28 to 0.45, staying on the standard of 0.1
