Fakultas Kedokteran Universitas Sebelas Maret Surakarta ABSTRAK Latar Belakang: Preeklamsia merupakan salah satu kontributor utama morbiditas dan mortalitas pada ibu dan janin. Bukti kuat ketidakseimbangan faktor anti-angiogenik dan angiogenik pada patogenesis preeklamsia salah satunya dijelaskan oleh adanya soluble Fms-like tyrosine kinase (sFlt-1) yang meningkat dan Vascular Endothelial Growth Factor (VEGF) yang menurun dalam sirkulasi darah. Pemberian VEGF 121 rekombinan sebagai pengobatan preeklamsia pada hewan coba memiliki kemampuan menurunkan kadar sFlt-1 dalam sirkulasi darah, secara klinis menurunkan tekanan darah, secara histopatologis perbaikan fungsi endotel plasenta. Jika terjadi disfungsi endotel maka pada permukaan endotel akan diekspresikan molekul adhesi, seperti vascular cell adhesion molecule-1 (VCAM-1). Penelitian ini bertujuan untuk membuktikan pengaruh pemberian VEGF 121 rekombinan terhadap perbaikan disfungsi endotel yang dinilai dari ekspresi VCAM-1 di plasenta mencit (Mus musculus) model preeklamsia. Metode: Penelitian eksperimental analitik dengan rancang bangun Randomized Control Trial (RCT) yang dilakukan pada bulan Februari sampai Maret 2016 di Kandang Hewan Percobaan dan Laboratorium Biomedik Fakultas Kedokteran Hewan Universitas Airlangga. Sampel penelitian sebesar 30 organ plasenta mencit dengan rumus replikasi dari Steel dan Torrie. Variabel penelitian yaitu pemberian VEGF 121 rekombinan sebagai variabel bebas, ekspresi VCAM-1 plasenta mencit bunting sebagai variabel tergantung. Parameter hasil pemeriksaan imunohistokimia untuk mengetahui ekspresi VCAM-1 pada pembuluh darah plasenta mencit. Hasil dinilai secara semikuantitatif menurut metode Remmele yang sudah dimodifikasi, dimana indeks skala Remmele (Immuno Reactive Score/IRS) merupakan perkalian antara skor persentase sel immunoreaktif dengan skor intensitas warna pada sel immunoreaktif. Analisis data menggunakan uji Kruskal Wallis dan Mann Whitney dengan SPSS (Software Package for social Science). Hasil: Penelitian ini menunjukkan bahwa VEGF 121 rekombinan terbukti memperbaiki disfungsi endotel plasenta mencit model preeeklamsia, terbukti dengan adanya perbedaan bermakna (p=0,035<0.05) yang menunjukan bahwa plasenta mencit preeklamsia dengan pemberian VEGF 121 rekombinan mempunyai nilai rerata ekspresi VCAM-1 lebih rendah (2,14) dibanding nilai rerata plasenta mencit preeklamsia tanpa VEGF 121 (2,94). Kesimpulan: VEGF 121 rekombinan terbukti dapat menurunkan ekspresi VCAM-1 di plasenta mencit model preeeklamsia. Kata Kunci: VEGF 121 rekombinan, VCAM-1, preeklamsia. John Arianto Sondakh. 2016. Family Medicine Magister Programme Sebelas Maret University Surakarta ABSTRACT Background: Preeclampsia is one of the major contributor to morbidity and mortality in the mother and fetus. Imbalance of pro-angiogenic and anti-angiogenic factors in preeclampsia strongly evidence can be explained by the presence of soluble Fms-like tyrosine kinase (sflt-1) increased and Vascular Growth Factor (VEGF) decreased in circulation. Provision of recombinant VEGF 121 as a treatment for preeclampsia in experimental animals have the ability to reduce levels of sFlt-1 in blood circulation, lowers blood pressure clinically, histopathologically improvement of endothelial function. Endothelial dysfunction will be expressed adhesion molecules in it surface, such as vascular cell adhesion molecule-1 (VCAM-1). This study aims to prove the effect of recombinant VEGF 121 towards improvement of endothelial dysfunction assessed the expression of VCAM-1 in placenta of preeclampsia-model mice (Mus musculus). Methods: This was an experimental analytic study design, randomized control trial (RCT) conducted from february to march 2016 in the biomedical laboratory of the Faculty of Veterinary Medicine, Airlangga University. Samples were 30 organs of the mice placenta using the formula replication of Steel and Torrie. The administration of recombinant vegf 121 as independent variables, and the expression of VCAM-1 mice placenta as the dependent variable. Immunohistochemistry examination is intended to determine the expression of VCAM-1 in the blood vessels of the placenta in mice. Results from each sample assessed semiquantitatively according Remmele-modified methods, where the Remmele index scale (immuno reactive score / IRS) is the result of multiplying the percentage of immunoreactive cells score to the color intensity immunoreactive cells score. Data analysis using Kruskal Wallis and Mann Whitney using SPSS (Software Package for Social Science). Results: This study demonstrates that recombinant VEGF 121 shown to improve endothelial dysfunction in the placenta of preeeclampsia-model mice evidenced by the significant difference (p = 0.035 <0.05), which showed that the preeclampsia mice by administration of recombinant VEGF 121 has lower average value of the expression of VCAM-1 (2.14) than the average value of preeclampsia mice without administration of recombinant VEGF 121 (2,94). Conclusion: The recombinant VEGF 121 is proven to reduce the expression of VCAM-1 in the placenta of preeeclampsia-model mice. Keywords: recombinant VEGF 121, VCAM-1, preeclampsia.
