ABSTRAK Latar Belakang Ulkus Kaki diabetik merupakan salah satu komplikasi kronis diabetes melitus . Proses perbaikan ulkus melibatkan keseimbangan antara TGF-β dan MMP-9. TGF-β memiliki efek pleiotropik salah satunya meningkatkan extra celullar matrix, sedangkan MMP-9 berkontribusi pada degradasi Extra Celullar Matrix. Propolis menghambat aktifitas sitokin proinflamatori dan meningkatkan ekspresi TGF-β dan menurunkan ekspresi MMP-9 sehingga dapat membantu mempercepat perbaikan ulkus kaki diabetik. Kurangnya bukti ilmiah peranan propolis topikal dalam meningkatkan ekspresi TGF-β dan menghambat ekspresi MMP-9 mendorong dilakukan penelitian ini. Tujuan Penelitian Menganalisis pengaruh propolis topikal 2,5%, 5% dan 10% dan menentukan dosis efektif propolis topikal 2,5%, 5% dan 10% terhadap perbaikan luka, ekspresi TGF-β dan MMP-9 pada mencit dengan luka kaki diabetik. Metode Penelitian Penelitian eksperimental dengan tikus sebagai hewan uji. Data disajikan dalam bentuk mean ± SD dengan nilai p < 0,05 dianggap signifikan. Hasil Penelitian Persentase Luas Ukuran Luka akhir : Kelompok DM : - 2,60 ± 10,33, DM + Propolis 2,5% : - 4,00 ± 14,97, DM + Propolis 5% : -11,33 ± 9,61dan DM + propolis 10% : -25,33 ± 13,06. Perbedaan dua rata rata: Kelompok DM – DM+Propolis 2,5% : 1,333± 10,32 (p <1,000) , Kelompok DM – DM+Propolis 5% :8,667 ± 14,96 (p <1,000) dan Kelompok DM – DM+propolis 10% : 22,667 ± 9,6 (p <0,018). Persentase Ekspresi TGF-β per 100 sel makrofag , pada kelompok Kontrol : 12,00± 2,61, kelompok DM : 44,50±8,36, Kelompok DM + Propolis 2,5% : 53,33± 2,73, Kelompok DM + Propolis 5% : 61,83± 3,87 dan kelompok DM + propolis 10% : 64,67± 2,42 l. Perbedaan dua rata rata: Kelompok DM – Kontrol : -32,50 ± 2,54 (p <0,001), kelompok DM – DM+Propolis 2,5% : -8,83 ± 2,54 (p <0,002), Kelompok DM – DM+Propolis 5% : -17,33± 2,54 (p <0,001) dan Kelompok DM – DM+Propolis 10% : -20,17 ± 2,54 (p <0,001). Persentase Ekspresi MMP-9 per 100 sel PMN, pada Kelompok Kontrol: 26,00± 4,43, Kelompok DM : 56,50± 4,04, Kelompok DM + Propolis 2,5% :51,17± 3,71 , kelompok DM + Propolis 5% : 47,17± 3,19 dan DM + propolis 10% : 44,67± 5,61. Perbedaan beda dua rata rata : Kelompok DM – Kontrol : -30,50± 2,62 (p <0,001), Kelompok DM – DM+Propolis 2,5% : -5,33 ± 2,62 ( p <0,051 ), kelompok DM – DM+Propolis 5% : 9,33± 2,62 (p <0,001) dan Kelompok DM – DM+Propolis 10% : 11,83 ± 2,62 (p <0,001). Kesimpulan Pemberian propolis topikal memperbaiki ukuran luka , meningkatkan ekspresi TGF-β dan menurunkan ekspresi MMP-9. Propolis topikal efektif memperbaiki ukuran luka pada konsentrasi 10%. Propolis topikal efektif meningkatkan ekspresi TGF-β pada konsentrasi5% .Propolis topikal efektif menurunkan ekspresi MMP-9 konsentrasi 5% . Ulcer repair process involves a balance between TGF-β and MMP-9. TGF-β has a pleiotropic effect of one extra boost celullar matrix, whereas MMP-9 contribute to the degradation Extra Celullar Matrix. Propolis inhibits the activity of proinflammatory cytokines and increased expression of TGF-β and decreased the expression of MMP-9 so as to help accelerate the improvement of diabetic foot ulcers. Lack of scientific evidence topical propolis role in increasing the expression of TGF-β and inhibit the expression of MMP-9 pushed to do this research. Research purposes Analyze the effects of propolis topical 2.5%, 5% and 10% and determine the effective dose of propolis topical 2.5%, 5% and 10% of the repair of injury, the expression of TGF-β and MMP-9 in mice with diabetic foot wounds. Research methods Experimental studies with mice as test animals. Data is presented in the form of mean ± SD with p value <0.05 was considered significant. Conclusion Topical propolis Award fix the size of the wound, increasing the expression of TGF-β and decreased the expression of MMP-9. Propolis topical effectively improve wound size at a concentration of 10%. Propolis topical increasing the expression of TGF-β effective at concentrations of 5% topical .Propolis lowered expression of MMP-9 is effective at concentrations of 5%.
