    Abstract

    EFFECT OF TNF-α LEVELS TO CLINICAL IMPROVEMENT AFTER ADMINISTRATION OF DEXAMETHASONE IN PATIENT PNEUMONIA Bobby Singh, dr.; Dr. Reviono, dr., Sp.P(K); Prof. Dr. Suradi, dr., Sp.P(K),MARS Master Program in Family Medicine, Post-Graduate Program, Sebelas Maret University Surakarta. Email : bobbyninu1520@gmail.com ABSTRACT Backgrounds: Pneumonia management consist of antibiotics and supportive treatments. Low dose corticosteroid can inhibit the transmission of proinflammation cytokines, accelerate the systemic resolution and lung inflammation in pneumonia. Dexamethason is the strongest synthetic corticosteroid. This study was conducted to assess whether there was an effect of TNF- α levels as a pneumonia infection biomarkers to clinical improvement in pneumonia patients. Methods: This study was a clinical trial with a quasi-experimental design. Subjects were patients with pneumonia who admitted in Dr. Moewardi Hospitals Surakarta since September until October 2015 which were taken by purposive sampling. Subjects were divided into two groups randomly, the first group received dexamethasone 5mg once a day (IV) for 5 days (n= 15), the second group only received empiric theraphy (n= 15): Examination of TNF-α levels assessed before and after dexamethasone administration and clinical improvement assessed by clinical stable criteria. Results: The levels of TNF-α serum was increased in control group, whereas for the treatment group was decreased. The decrease of TNF-α in treatment group obtained not significant results of statistical analysis (p=0.570). The difference of clinical improvement between the treatment and the control group obtained p value = 0.042, which means statistically significant. Correlation between TNF-α levels and clinical improvement was not significant statistically (p=0.128). Conclusions: Dexamethasone hadn’t a role in the decrease of TNF-α serum levels of pneumonia patients. Dexamethasone had a role in the clinical improvement of pneumonia patients. There was no correlation between the decrease of TNF-α serum levels and clinical improvement after 5 days dexamethasone administration in pneumonia patients.
