    Abstract

    Background: Sepsis is still a cause of high mortality worldwide. Sepsis patients sometimes require sedation in the ICU. TNF-α is one of the major mediator of sepsis syndromes. Dexmedemetomidine and clonidine are α-2 agonists that are believed to have the ability to reduce levels of TNF-α and can be used for sedation in patients with sepsis. Objective: To analyze the difference of dexmedetomidine and clonidine on TNF-α serum level in wistar rats with septic polymicrobial model. Methods: This research is categorized as laboratoric experimental with post test only control group design. As many as 35 wistar rats were being adapted for 7 days and then performed cecal inoculum of 200 mg/kg/day for 3 days but only 33 rats were found to fulfill the criteria of sepsis then they randomly divided into three groups. The first group is a control group, this group is septic rats which were given 0.5 ml of 0.9% NaCl intravenously every 6 hours. The second group is septic rats which received 10 mcg/kg of dexmedetomidine intravenously every 6 hours. The last group is septic rats which received 15 mcg/kg clonidine intravenously every 6 hours. After 24 hours, 2 ml of the blood was drawned and then examined for TNF-α serum level. During the treatment, there was 4 rats that died, 2 rats in the control group, 1 rat in dexmedetomidine the group, and 1 rat in clonidine group. Data abnormality was analyzed with Shapiro Wilk and homogensity test was done with Levene test. Data analysis was continued with Kruskal Wallis and Mann Whitney test. Significance level is p < 0,05. Result: There was a significant difference of TNF-α serum levels in dexmedetomidine group (11.493 ± 4.490 pg / ml) compared with the control group (49.855 ± 16.639 pg/ml) with p=0,001. There was a significant difference of TNF-α serum levels in clonidine group (13.760 ± 5.525 pg/ml) compared with the control group (49.855 ± 16.639 pg/ml) with p=0,001. But, the dexmedetomidine (11.493 ± 4.490 pg / ml) and clonidine groups (13.760 ± 5.525 pg/ml) were not statistically different with p = 0.771. Conclusion: There are significant differences in TNF-α serum levels between dexmedetomidine with control group and between clonidine with control group, but there is no significant difference between dexmedetomidine and clonidine group in wistar rat with septic polymicrobial model.
