    Abstract

    Introduction. Tuberculosis is still a major problem in the health world. Proper diagnosis to find Mycobacterium tuberculosis (M.tb) early is indispensable in breaking the chain of transmission. The diagnosis of pulmonary tuberculosis in Indonesia is still based on microscopic examination of acid fast bacilli (AFB) in sputum and culture in the M.tb media Lowenstein Jensen (LJ). Microscopic smear has limitations, sensitivity and microscopic smear varies greatly depending on the workload, personnel skills in reading preparation. Culture takes a long time is 2-8 weeks. Mycobacterium tuberculosis antigen (TBAg) rapid test kit is a test that is fast, easy, practical, and does not require special skills. This test detects antigens secreted M.tb genes that code RD-1 (Region of Difference 1), RD2 and RD3, where this region is eliminated in all strains of Mycobacterium bovis BCG. This study aims to determine the diagnostic value of a rapid test kit TBAg on sputum and serum samples for the diagnosis of tuberculosis (TB) lung. Method This study used a diagnostic test design. The samples were sputum and serum from 50 patients suspected of TB. This study uses the gold standard culture in the M.tb LJ media. Sputum and serum samples suspected pulmonary tuberculosis obtained from patients treated with the first symptoms of pulmonary TB in BBKPM Surakarta. Each specimen was examined TBAg rapid test kits. Results Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (PLR), negative likelihood ratio (NLR) and the accuracy of sputum samples, respectively for 72.2%, 85.71%, 92 , 86%, 54.55%, 5.06%, 0.32% and 76%. As for the serum samples respectively by 19.11%, 92.86%, 87.50%, 30.95%, 2.72%, 0.87% and accuracy of 40%. Conclusion In general, antigen detection by rapid test kit TBAg using sputum samples have better diagnostic value than serum samples. So it can be considered in healthcare facilities that do not have trained personnel to mikrokopis smear examination in order to use TBAg rapid test kit to aid in the diagnosis of pulmonary tuberculosis due to a fairly good specificity.
